These production back-up insurance invoices were production made for The War with Grandpa starring Robert De Niro, Uma Thurman and Christopher Walken. There are three different invoices, one from State Farm which was used in the scene and then Allstate and one with no insurance company name. A couple of the State Farm and one of the Allstate invoices are actually signed, possibly by Rob Riggle during one of the takes because he does sign this form on screen.
